The CHIP Model

Community & Home Independence Partners offers occupational therapy with specialization in neurological and cognitive symptom management using the following model:

No Stress, No Commitment Consultation

During a 15-minute free phone or via video* consultation, we will discuss your needs and what to expect from our services, including available assessment and interventions, funding, and availability for treatment.

Initial Evaluation & Advanced Assessment

Conducted in your home or via video*, the initial evaluation will consist of interview, observation, and basic assessment of current physical, cognitive, and social functioning. We will identify your highest priorities and set personalized goals to work towards during our time together & beyond. If further assessment is warranted past initial evaluation session, we will schedule sessions in the most fitting environment (home, community, or video) to complete thorough identification of needs and, together, we will develop the best way to approach your treatment.

Home-Based Intervention & Ongoing Assessment

Individual services may be provided in your home or via video*, providing training or developing strategies for greater independence with desired tasks. This intervention may focus on strengthening and skill-building for activities completed in the home setting, identifying and training on tools or equipment to limit need for assistance, or home environment assessment and recommendations to reduce risk of falls.

Community-Based Intervention & Ongoing Assessment

In most cases, community-based services are not covered by insurance funding. However, as community accessibility and engagement are integral to a full life for many, Community & Home Independence Partners offers a unique opportunity for a skilled assessment of safety, accessibility, and independence in various community settings - including, but not limited to, grocery stores, shopping centers, restaurants, neighborhood parks, local entertainment venues and attractions. 

Caregiver Education

In some circumstances, caregivers may desire their own sessions to receive education and/or training. These services are available and encouraged as a support to adjustment to a loved one’s changes in ability and/or a shift in the caregiver’s role within the relationship.

*All video services are provided via our HIPAA compliant secure telehealth platform, when appropriate. Eligibility will be discussed during initial consultation.
