What is Community & Home Independence Partners?
Community & Home Independence Partners provides individualized occupational therapy services to develop plans to address cognitive or physical barriers to living the life you desire. Our services are available to anyone experiencing new or long-standing difficulties in completing desired activities, at home or out in public places, due to mental health or neurological conditions. We also educate and support caregivers in providing care to loved ones with less stress and greater harmony in the home.
From recommendations on home access modifications to the best tricks to manage your to-do list, no issue is too small or too large to tackle together.
Community & Home Independence Partners is currently in-network with Medicare.
We may also serve as an out-of-network provider for billable occupational therapy services.
For all services, we accept private pay, and we can create a Superbill for your services, providing you an opportunity to submit for reimbursement from your insurance company.

Our free, 15-minute consultation will provide you with a brief overview of our areas of expertise and ask you to share your current needs with guiding questions. "Community & Home" spaces are anywhere in the local area where you need or would like to spend time.
Community spaces may include stores, restaurants, parks and playgrounds, street fairs or farmer's markets, entertainment venues (movie theaters, concert venues, sports arenas, etc), church, libraries, a friend's home, work, and more. Community activities require skills to plan, prepare, and participate in tasks such as shopping, attending a family gathering, going to a church function, seeing a show, or returning to work or volunteer opportunities.
Home independence includes activities like bathing, dressing, toileting, cooking, cleaning, financial management, medical management, transportation arrangement, and parenting or caregiving.
Independence in these spaces includes safe movement, good planning and follow-through, use of the most effective tools and strategies to simplify a task and make it more safe, and adequate social and organization skills to engage in the activity with success.
